How do I book an appointment?

To book an appointment call us, 01273 480888, or send a request online through Anima

Our aim is to connect you to the right person for your care, first time.

To do this you will need to answer a series of questions online/over the phone. Giving our team the information they need to process your request.

What happens next?

This will depend on your health needs and the number of appointments available.

If you are generally well, but need on the day advice we will book a telephone appointment. This may be with a GP, Paramedic, or other healthcare professional that is best suited to your care needs. If they need to see you in person they will arrange a time for you to come in and see them face to face.

If you have an ongoing conditions or more complex care needs we aim to get you to someone who knows you best. Whether this be your GP or a member of their GP team.

 

When will my appointment be?

For many patients an appointment will be offered the same day.

Yet, not all issues need resolving the same day. In some cases, where appropriate, we will offer an appointment within a week or up to 6 weeks ahead.

Who will I see?

Some conditions may also be better dealt with by our wider healthcare team. For example, we offer direct booking with our physiotherapists. Who can deal with muscle, ligament, tendon or joint pain.

How can I cancel or amend an appointment?

To cancel an appointment:

• Call 01273 480888, and select Option 2. Please leave a message, this must include your name, date of birth and reason for the cancellation.
• Log in to your Online Services account
• Through the NHS App

We ask that patients do not email or use the contact form on our website to cancel appointments.

How can I book a Home Visit?

Where possible, we prefer to see you at the surgery. Here we have the right equipment and staff to deal with your problem.

If you are housebound or too ill to come to the practice, we may arrange a home visit.

To assist us in allocating visits to our team please call us before 11:00am where possible.

The clinician will call you beforehand, this may be a GP or a Paramedic. They will assess your need for a home visit and confirm times and arrangements for access. Majority of our home visits our dealt with by our paramedic team, where necessary we will appoint a GP.

On occasion we may arrange to visit you on another day.

It may also be more appropriate to arrange a visit from:

The Community Nurses
Health and Social Care Connect (HSCC)

The clinician can arrange this for you.

Depending on your symptoms we may suggest you contact 111, 999 or go direct to A&E without delay. These may be for clinical reasons, such as suspected stroke or heart attack.

Not having access to transport is not a suitable reason to request a home visit. To assist you, there are transport options available to patients through the CTLA.
