Medication Reviews

Keeping Your Medicines Safe

If you take regular medicine, you may need a medication review. This helps make sure your medicine is still right for you and working safely.

This can be a blood test, check-up, or questionnaire. These help us to spot any side effects, or unexpected reactions.

Why Do I Need a Medication Review?

A medication review helps to:

✔ Check if your medicine is still the best choice
✔ Make sure your medicine is working well
✔ Find and manage any side effects or unexpected reactions.
✔ Change your dose if needed
✔ Answer any questions you have

If your medicine needs blood tests or check-ups, these will also be reviewed.

What Are High-Risk Medicines?

Some medicines can cause serious side effects.

In these cases, we need to do some extra checks.

Common medications include those for:

ADHD: Methylphenidate, Lisdexamfetamine
Blood thinners: Warfarin, DOACs
Diabetes: Insulin, some tablets
Immune system: Methotrexate, Azathioprine
Mental Health: Lithium
Thyroid: Levothyroxine, Carbimazole

Other medicines that need extra monitoring

Some Blood Pressure Medications: Ramipril, Losartan, Indapamide
Diuretics ‘water tablets’: Bumetanide, Furosemide, Spironolactone

Due to how both these work, we need to regularly check your kidney health, as well as your blood pressure.

Why Do I Need Blood Tests or Check-Ups?

Regular tests help to:

✔ Make sure your medicine is working well
✔ Find side effects early
✔ Change your dose if needed
✔ Keep your liver and kidneys healthy
✔ Spot early signs of serious problems such as infections or bleeding

What Happens If I Miss a Review or Test?

Missing a review or test could put your health at risk.

Your GP may need to reduce the quantity per prescription or pause your medicine until you have the check-up.

How Will I Know When I Need a Review?

We will text, call, or send a letter when you are due for a review or test. If you think you are overdue, please contact the surgery to book an appointment.

Your safety is important. Thank you for working with us to keep your treatment safe.
