Our Wellbeing Team includes Social Prescribers, Mental Health Support Coordinators, a Wellbeing Advisor, a Mental Health Practitioner and a dedicated Care Coordinator.
They work alongside our clinical teams, offering non-medical support and advice to patients, their families and carers to improve patient wellbeing and to tackle social isolation. They work with many local groups and charities, connecting patients to organisations within the community.
Some of our more experienced staff have been appointed as Care-Coordinators. Offering personalised care, they can assist individual patients and carers with non-clinical issues, identify unmet needs and signpost to community or voluntary services.
We have appointed Care Coordinators for Dementia and Frailty, Mental Health & Wellbeing, Cancer, Palliative Care, Refugees, Frequent Attenders, Nursing & Care Homes, Adult & Child Safeguarding and Learning Disabilities.
