Sick note for less than 7 days

If you have been ill for 7 days or less you can complete a Self-Certification form yourself.

Many employers have their own form. If your employer doesn’t have their own form you can complete the form below. Please print it, fill it in and hand it in to your employer. You do not need to see a clinician.

Sick note after 7 days

If you are seeing a clinician, and they suggest you need a sick note, they will issue it at that time.

You can request a sick note, or a further sick note if yours is due to expire, through:

•  Calling 01273 480888, option 1, after 1pm

The clinician may:

•  Agree to your request and issue without need for contact.
•  Ask that you to arrange a review to discuss your request and any return to work plans.

Most certificates are sent via email or text to your mobile phone. You can then send it to your employer as required. We will issue paper copies upon request, but this may not be the same day.

Private sick notes

Some employers may insist on a sick note regardless of the 7 day self-certification rule. A private sickness certificate issued under these circumstances involves a charge of £25.
