Am I Eligible?

Our Adult Social Prescriber is available to patients aged 21 to 64 years old.

Is Social Prescribing right for me?

Are you:

• Feeling overwhelmed by your health issues?
• Worried about housing, or money, and feel like it's starting to affect your health?
• Recently diagnosed with a health condition and looking for support?
• Feeling isolated or lonely? Would you like to meet up with some like-minded people?
• A carer for someone and feel like you are not coping?
• Feeling like you don't have much quality of life at the moment?
• Looking to be healthier, more active and eat better?

How can I contact the Social Prescriber?

You can contact our Social Prescriber directly. Simply submit a request online, via Anima. Or, call 01273 480888, select option 2, and complete a request with one of our receptionists.

The information we receive from you will give us a little insight into:

• your background,
• interests, and
• what you are struggling with at the moment.

You may also be referred to the Social Prescribing Team by one of our GPs, Nurses, or Paramedics.

Please Note: This is NOT an urgent service. Our Social Prescribers work part-time,  and demand is high. There is currently a 2-3 week wait for those seeking this service.

What will happen next?

Our Adult Social Prescriber will be in touch. They will work with you to create a personalised care plan.

Offering both face-to-face and telephone appointments. They aim to find the right time to suit you.

They will find out what is most 'important to you.' Then search to find groups, activities, or services that can best support you.

Once they have found the right place for you, you may find you no longer need the service. You can always come back if you need to again in the future.

Can Social Prescribers provide medical advice or treatment?

Social Prescribers cannot:

• diagnose medical conditions
• discuss or issue medication
• discuss or give test results

You will need to contact Foundry via Anima to discuss these issues with a member of the Clinical Team.
